Course Overview:
This training equips community health workers, home health aides, care coordinators, social service professionals, and other frontline staff with the practical knowledge and skills to:
Conduct evidence-based fall risk screenings.
Perform standardized functional mobility assessments.
Identify home and environmental fall hazards.
Recognize common medical and behavioral fall risk factors.
Document assessment findings appropriately.
Connect older adults with evidence-based programs and community resources using Missouri-specific referral pathways.
Learning Outcomes:
By the end of this training, you will be able to:
- Conduct evidence-based falls risk screenings using validated tools.
- Perform standardized functional mobility assessments safely and accurately.
- Recognize intrinsic, extrinsic, environmental, and behavioral fall risk factors.
- Identify home hazards and recommend practical safety modifications.
- Document assessment findings using best practices.
- Connect older adults with healthcare providers, evidence-based programs, and community resources using Missouri referral pathways.
- Support older adults in reducing their fall risk and maintaining independence.
